For emergencies please direct employee to the nearest Emergency Room or Clinic. If possible ensure Employee leaves with the Verification of Employment for Reported Injury or Illness & the First Fill Medication Card forms and contact Administration immediately.

ALL injuries must have a First Report of Injury form completed and returned to Sherry Brinlee within 24 hours.
Please refer injured employee directly to Sherry Brinlee for any further questions or issues regarding any workers’ compensation injury. Alert Administration immediately if employee misses any time, returns to work, or if there are any questions or concerns.
